My Doctor’s Notes Were Typed By A Medical Dictation Service

I was a little bit curious how the chart notes and all of the paper that ends up in my medical record was processed. I knew that after each appointment with a patient, the doctor goes in to his or her office and speaks in to a small microphone in order to give their case notes verbally. What I did not know until recently, is that those tape recorded bits of speech are then processed by a medical dictation service which has people working in all locations. They listen and type up what was said and that is how the notes get in the chart.
